Subject: Operations Manual Update: Chapter 4, Part 2 - Youth Program Activities & Attachments

Purpose:

The purpose of this issuance is to notify Youth Service Provider of revisions to the following policies: Chapter 4, Part 2 – Youth Program Activities & Attachments:

·Chapter 4, Part 2 – Youth Program Activities

  • Corrective Action Policy

  • Corrective Action Plan

  • Incentive Log and Receipt Form

  • Progress Report Form

  • Stipends Log and Receipt Form

  • Supportive Services Log and Receipt Form

  • Work Experience Checklist

  • Youth Participant File Monitoring Checklist

Major changes have been made to the above chapters, including, but not limited to:

Chapter 4, Part 2

  • Individual Employment Plan/Service Strategy (IEP/ISS)

    • Removal of the ISS as an attachment

    • The ISS will be located in CalJOBS

  • Addition of the Objective Assessment Summary

    • The Objective Assessment will be located in CalJOBS

Significant changes have been made to the above chapters and attachments including but not limited to:

Chapter 4, Part 2

·Refresh of the chapter and attachment formatting

·Re-ordering of chapter sections for ease of use

·Clarification of documentation process

  • Updates and clarification of procedures

·Update of source documentation references

·Removal of outdated attachments

Action:

Effective March 1, 2018 all Youth Service Providers are required to follow the guidelines outlined in the updated chapters and attachments. This issuance replaces and supersedes the current chapter and attachments.
